Privacy and cookie policy of midwifery practice beWonder

General

Midwifery practice beWonder processes a range of your personal data. This is necessary for your medical treatment and for settling the costs of it. Processing this data may also be necessary in order to meet a legal obligation — for example the duty to report an infectious disease under the Public Health Act.

The General Data Protection Regulation (GDPR) came into force on 25 May 2018. Since that date the same privacy legislation applies throughout the European Union. Alongside the general rules there are specific rules for privacy in healthcare, set out among other places in the Dutch Medical Treatment Contracts Act (WGBO).

Midwifery practice beWonder is therefore obliged to handle your personal data with care. This privacy policy sets out your rights and our duties as they apply under the GDPR and the WGBO.

The practice's duties

Midwifery practice beWonder is responsible for processing all the personal data it receives about you. Certain duties come with that, and we meet them as follows:

  • As a rule your data is processed only for providing care, for efficient management and policy, and to support scientific research, teaching and public information. In principle no processing takes place for other purposes.
  • You are informed that your personal data is being processed. This may be through the registration form, a leaflet or our website.
  • All staff at midwifery practice beWonder are bound to treat your personal data in strict confidence.
  • Your personal data is properly protected against unauthorised access.
  • Once your treatment has ended, your personal and medical data is kept for a further 15 years.

Automated decision-making

No automated decisions are taken about individuals, certainly not on matters that could have far-reaching consequences for them. This refers to decisions taken by computer programs or systems without a person — a member of beWonder staff, for instance — being involved.

Midwifery practice beWonder uses the Orfeus software to record personal data.

Your rights

You have the following rights:

  • The right to know whether personal data about you is being processed and, if so, which data.
  • The right to see that data and to receive a copy of it, in so far as this does not harm another person's privacy.
  • The right to have data corrected, added to or removed, should that be necessary.
  • The right to ask for your medical data to be destroyed, in whole or in part. Such a request will be granted if keeping the data is not of considerable importance to someone else and/or it does not have to be kept under a statutory provision.
  • The right to add a statement of your own, of a medical nature, to your file.
  • The right in certain cases to object to your data being processed.

If you wish to exercise your rights, you can let midwifery practice beWonder know in person, by letter, by email or through the enquiry form on the website.

The registration form

By answering the questions on the registration form as fully as you can, you help us a great deal in finding your file and in protecting your privacy. Everything you tell us is of course treated in strict confidence. It is worth knowing that medical data is in principle kept for at least fifteen years. That is a legal requirement.

Sharing your personal data with others

Staff at midwifery practice beWonder are obliged to treat your personal data in strict confidence. That means, for example, that we always need your express consent before passing your personal data on to anyone else. It is only ever passed to people directly involved in carrying out the treatment agreement you entered into with us — the midwife, your family doctor, the child health clinic, the maternity care service, the gynaecologist and the paediatrician. Anonymised data may also be supplied to bodies responsible for monitoring quality, such as PRN and Peridos.

There are a few exceptions to this rule. A care provider's duty of confidentiality may be broken on the basis of a statutory provision. The same applies if there is reason to fear serious danger to your health or that of someone else. Recorded data may also, where necessary, be exchanged with other care providers verbally, in writing or electronically.

Whenever we request information about you from a third party, a consent form signed by you is always sent with the request.

How we protect personal data

Midwifery practice beWonder takes the protection of your data extremely seriously and does everything it can to prevent misuse, loss, unauthorised access, unwanted disclosure and unauthorised alteration. If you feel your data is not properly protected, or you have any indication of misuse, please contact us.

A question or a complaint

Do you have a question or a complaint — about how we handle your medical data, for instance, or about who we share your data with? Your midwife will gladly talk it over with you. Midwifery practice beWonder would also point out that you can always lodge a complaint with the national supervisory authority, the Dutch Data Protection Authority.

Privacy and cookie statement

Midwifery practice beWonder attaches great importance to the privacy of visitors to this website. To safeguard your privacy as well as we can, we work to a number of core principles.

  • Informing you — We want to tell you as clearly as we can how and why we process personal data. That is what this privacy statement is for.
  • Collecting less — We take care to limit our collection of personal data to what is genuinely needed for the website to work properly and for the services you have asked for.
  • No marketing based on personal data — We do not hand over personal data for marketing purposes.
  • Securing — We take appropriate security measures to protect your personal data, and we require the same of parties that process personal data on our behalf.

Use of personal data

By visiting the website and using the services offered on it, you leave certain data with us. That may be personal data. We store and use only data we have obtained directly from you, or data where it was clear to you when you supplied it that it was being given to us to process.

Depending on the service you use, we may collect the following:

  • name, address and place of residence
  • email address
  • telephone number
  • date of birth
  • name of your family doctor
  • name and policy number of your health insurer
  • citizen service number (BSN)
  • details of earlier pregnancies
  • first day of your last period

For each service, the website states which data we collect.

This data is used for the following purposes:

  • creating your account
  • scheduling an appointment
  • keeping track of preferences you have given
  • carrying out other services you have asked for.

Security

We have taken security measures to limit misuse of, and unauthorised access to, personal data as far as possible. We keep statistics on our website, but always entirely anonymously. No statistics are kept on parts of the website where your health data is exchanged.

How long we keep data

Unless a legal obligation stands in the way of deletion, your personal data is never kept longer than is needed for the purposes described in this privacy statement or on the website. But, again: medical data must in principle be kept for at least fifteen years. That is a legal requirement.

Third-party websites

This privacy statement does not apply to third-party websites reached through links on this website. We cannot guarantee that those third parties handle your personal data reliably and securely. We therefore advise you to read the privacy statement of such websites before using them.

Cookies

We use cookies on this website. A cookie is a small computer file sent along with pages of this website and then stored by your browser on your computer's hard disk. On a later visit to this website, the information stored in it can be sent back to our servers — or to the servers of the third parties concerned.

Google

Through our website, cookies from the American company Google are placed on your computer as part of Analytics. We use this service to keep track of, and receive reports on, how visitors use the website.

We have concluded a data processing agreement with Google. The information Google collects is anonymised as far as possible, and we have IP addresses anonymised as well. We have not given Google permission to use the information obtained for other Google services. We have no influence, however, over how Google and/or third parties use the data. Google may supply this information to third parties if legally obliged to do so, or in so far as third parties process the information on Google's behalf.

The information Google collects is transferred to and stored on servers in the United States. Google has declared that it abides by the Privacy Shield principles and has joined the US Department of Commerce's Privacy Shield programme. This means an adequate level of protection applies to the processing of any personal data.

Permanent cookies

Once you have visited our website, permanent cookies allow us to recognise you on each subsequent visit, so the website can be set up to suit your preferences. And once you have given permission for cookies to be placed on your computer, we can remember that by means of a cookie. The advantage is that you do not have to state your preferences over and over again, which saves you time and makes our website more pleasant to use.
